S.F. No. 2028 directs the commissioner of human services to develop a pilot project with the Center for Victims of Torture (CVT) to provide mental health targeted case management using an integrated care model at three service sites. The services shall be funded fully with state funding and reimbursed at a rate of $695 per member per month. Individuals who are eligible for medical assistance, are eligible for targeted case management, and are being served by the CVT are eligible to participate in the pilot project. Pilot project participants will be evaluated using the social functioning tool developed by the CVT. In addition, the commissioner and the CVT will evaluate whether the social functioning tool can be adapted for use with the broader medical assistance population. $1,000,000 is appropriated onetime in fiscal year 2020 to fund the start-up and additional operating costs of the pilot project, and a separate amount shall be appropriated onetime for the commissioner’s evaluation of the social functioning tool’s possible use with a broader population.
